The completion or termination of the study is a change in activity and must be reported to the IRB. Although subjects will no longer be "at risk" under the study, a final report to the IRB allows it to close its files as well as providing information that may be used by the IRB in the evaluation and approval of related studies.

Investigators may submit a Closure Request Letter to the IRB and complete a continuing review form or at the time of continuing review submit the documents for continuing review and a Closure Request Letter.

The IRB Chair or designated IRB Member(s) will review the request for closure by expedited review and closed effective the date of Chair review. The IRB staff will then notify the IRB at the next scheduled IRB meeting. It will be presented on the full board as part of the “expedited activity report”.
