The HSRC will be meeting the 3rd Tuesday of each month during the Spring Semester of 2008. Full Board review applications need to be submitted to the office by the 1st of the month.

Exempt/Expedited can be submitted at anytime.

  1. Protocol Manager is no longer functioning as we move toward an electronic system, InfoEd. Protocol Numbers may no longer, therefor, be requested on-line. When you submit your application materials in hard copy, you will be notified of the protocol number assigned to your protocol.
  2. To locate and download application materials, See the "How To....." link on the home page. Review the definitions for Exempt, Expedited, and Full Board submissions to determine which application process best suits your proposed human subject research.
  3. Download the appropriate application form and attachments by clicking the link and saving the document to your local hard drive, and then opening the document in MS Word. You may then type in and save the document. Print the document for submission to HSRC.
  4. Include with your submission to HSRC all of you data collection tools (e.g. questionnaires, surveys, interview guides, list of variables, etc.). As well as your recruitment materials (e.g. recruitment letter, email script, telephone script, in-person or face-to-face scripts). Label all documents with the PI's name in the header.
  5. Complete the educational requirements. For HSRC, this is the online CITI Human Subjects Protection Training (for all research personnel on the application)
  6. Mail or drop off you application materials to the HSRC office, 3rd Floor, Suite 300, Lawrence Street Center.
  7. Note: If you are submitting for a Full Board Review, you must submit 15 collated copies of your application materials by 5:00 pm on the 1st day of the month in which you wish have a review. For Exempt/Expedited review, please submit one (1) copy of your application. Exempt/Expedited applications are reviewed in the order in which they are received by a member of the board and, therefore, are not affected by the full board meeting schedule.
